Beaverton School District students in grades 6, 8 and 11 will take an anonymous online health survey in November, the district announced.
The Student Health Survey (SHS) is a statewide questionnaire sponsored by the Oregon Health Authority (OHA) and the Oregon Department of Education (ODE). It asks students about mental and behavioral health, substance use, sexual and reproductive health risks, school climate and what puts students at risk or supports them, according to the OHA.
The survey also collects demographic information, including sexual orientation and gender identity, which the OHA says helps identify health disparities among youth.
The OHA calls the SHS the only statewide source of youth health data collected directly from students, according to its data portal. Schools and state agencies use the results to shape health and education policy.
Participation is voluntary. Students can skip any question or opt out entirely. Parents who do not want their child to take the survey should contact their school's main office, the district said.
Responses are confidential. No identifying information is collected, and answers cannot be linked to individual students.
Copies of the survey questions are posted on the district's website under Annual Surveys. State and county results from the 2025 survey, including a Washington County report, are available through the OHA's results page.
